Healthy Choices is a Kiowa County voucher food program to improve the diets of low-income families while providing an additional food supplement resource to the current programs being offered by the Department of Human Services.
The goal of Healthy Choices is to address food insecurity in Kiowa County by contributing to household food budgets for the purchase of whole natural healthy foods from the fruits, vegetables, dairy, and protein food groups.
To apply for the Healthy Choices program, pick up an application at 1305 Goff Street, Eads, Colorado, or print the application from the link below. Return the completed application along with a copy of your most recent pay stub or benefit statement to the Kiowa County Commissioner's Office, 1305 Goff Street, PO Box 100 Eads, CO 81036. Proof of income is required to process an application.
